ucontext.h: Expose __UCONTEXT_SIZE to userland.

But don't expose __CTASSERT(__UCONTEXT_SIZE == sizeof(ucontext_t)) to
userland.

- __UCONTEXT_SIZE will be needed soon by libc and signal trampolines
  in order to version ucontext so we can safely expand it with
  architecture extensions.

- __CTASSERT is not safe to use in header files in arbitrary
  compilation environments (although in the NetBSD kernel build it is
  safe).  Exposing the __CTASSERT in a header file used by userland
  appears to have the side effect of breaking the Firefox build in
  rustc, though the mechanism isn't entirely clear.

Let's see if this makes a difference -- if it works, great; if not,
maybe it will help us to narrow down what's happening with rustc and
Firefox.

Move __UCONTEXT_SIZE inside _KERNEL like before

Fix pkgsrc/lang/rust runtime SIGILL error during pkgsrc/www/firefox build.

If <sys/mcontext.h> does not define __UCONTEXT_SIZE, define it as
sizeof(ucontext_t).

Define the _UC_* flag bits before including <sys/mcontext.h>.

Clean up the <sys/ucontext.h> <-> <machine/mcontext.h> interface up
a little:
- Define _UC_MD_BIT* constants for the available machine-dependent bits,
  and use those constants to define the machine-dependent bits as well
  as the machine-independent bits that have machine-dependent values.
- Explicitly generate an error if _UC_TLSBASE, _UC_SETSTACK, or
  _UC_CLRSTACK are not defined by <machine/mcontext.h>.

Move CTASSERT a few lines down so as to not collide with the CTASSERT
in rtsock.c (relevant for compilers without __COUNTER__).  Yes,
it's a really cheap tweak.  Tweak better if it tickles your tweakybone.

setcontext() used to be incompatible with -lpthread since it affected
the TLS pointer, therefore wrecking the pthread environement.

Some ports had _UC_TLSBASE flag or equivalent (_UC_UNIQUE on alpha)
that controlled whether setcontext() would change the TLS pointer.
This change let libpthread override setcontext() with its own version
that unsets _UC_TLSBASE, enabling safe usage of setcontext() with
-lpthread.

We also have the following required changes here:
- rename alpha's _UC_UNIQUE into _UC_TLSBASE
- add _UC_TLSBASE definition in header file for all ports
  (powerpc, sh3, sparc and sparc64 lack the implementation for now)
- introduce a libc stub that can be overriden for setcontext()
- modify MD libcs swapcontext() implementations so that they use the
  setcontext() libc stub instead of doing a plain system call.

While we are there:
- document various MD _UC_* flags in header file
- add libc and libpthread tests for swapcontext() behavior
  (hopefully helpful to spot MD problems introduced with this change)

Future work:
- Deciding whether kernel support or _UC_TLSBASE should be added for
  powerpc, sh3, sparc and sparc64 is left to portmasters
  sparc64

Calling _lwp_create() with a bogus ucontext could trigger a kernel
assertion failure (and thus a crash in DIAGNOSTIC kernels). Independently
discovered by YAMAMOTO Takashi and Joel Sing.
To avoid this, introduce a cpu_mcontext_validate() function and move all
sanity checks from cpu_setmcontext() there. Also untangle the netbsd32
compat mess slightly and add a cpu_mcontext32_validate() cousin there.
Add an exhaustive atf test case, based partly on code from Joel Sing.
Should finally fix the remaining open part of PR kern/43903.

Fix ucontext32_t on AMD64. Add optional compile time assertions for
ucontext_t and ucontext32_t to ensure that they don't change.
Provide the constants for AMD64, ARM, i386, and M68K.

Merge wrstuden-revivesa into HEAD.

Add getucontext32_sa() and getucontext_sa(), and use them. They
are sa-specific versions of getucontext{,32}(). The main difference
is that they do NOT require the p_lock be held. We report an empty
signal mask, both to avoid needing p_lock and because the process
signal mask is almost always 0 for SA.

add a no-op default value for _UC_UCONTEXT_ALIGN if MD code doesn't set it.

Define a few 32bit compat types (if COMPAT_NETBSD32, not exported to
userspace). Having them here is both good and bad. Good because they're
close to the actual native ones, bad because it exposes things out
of compat/netbsd32. However, putting the exclusively in the latter
requires a lot of reshuffling in the includes there, so this will
do for now.

Remove pointless inclusion of <sys/cdefs.h> (a pre-ANSI-KNF leftover).

Merge the nathanw_sa branch.

* Include <sys/sigtypes.h> rather than <sys/signal.h> in <sys/ucontext.h>.
* Define _UCONTEXT_TO_SIGCONTEXT() and _SIGCONTEXT_TO_UCONTEXT()
  macros for converting a ucontext -> sigcontext and back again.
  These macros in turn use machine-dependent macros _MCONTEXT_TO_SIGCONTEXT()
  and _SIGCONTEXT_TO_MCONTEXT() provided by <machine/signal.h>.

  The conversion process is not 100% accurate, but should be close enough.

  Also note that the mcontext conversion may not be enough for all platforms
  (m68k is a good example of this).  These macros should be used only if
  you really know what you're doing.

Also protect the inclustion of signal.h with _STANDALONE

Include <machine/mcontext.h> after <signal.h> to get any signal definitions
needed by mcontext (such as struct sigcontext).
