Assorted minor cleanups, no functional change:
   - u_int* -> uint*
   - don't make private typedefs of system structures
   - use curses TRUE and FALSE only with curses booleans, otherwise
     true and false;
   - includes cleanup
   - group globals in extern.c by functionality
