Sync with HEAD

- build the gthr*.h files to be installed on the fly, instead of
  having dozens of copies of the same thing in the tree.
- don't fetch any gthr*.h files.
- delete ancient gthr-tpf.h that was removed in GCC 4.8 or so.

this should fix the call_once tests for real now.

mknative-gcc for GCC 7 and earm targets.

rebuild mknative GCC 6.4 for these targets:

generate mknative-gcc for GCC 5.3 and all the arm targets.
minor regen for x86 and sparc*

Rebase to HEAD as of a few days ago.

sync with head.

for a reference, the tree before this commit was tagged
as yamt-pagecache-tag8.

this commit was splitted into small chunks to avoid
a limitation of cvs.  ("Protocol error: too many arguments")

file gthr-single.h was added on branch yamt-pagecache on 2014-05-22 16:53:30 +0000
