Remove directory, requested by mrg in ticket #123:

This directory is not used on this branch.

have it derived from the HAVE_GCC value so it does not need to
be modified for newer GCCs.

remove remaining makefile support for GCC < 45 that i found.

for a reference, the tree before this commit was tagged
as yamt-pagecache-tag8.

this commit was splitted into small chunks to avoid
a limitation of cvs.  ("Protocol error: too many arguments")

update gcc.old from GCC 4.5 versions that build with EXTERNAL_GCC_SUBDIR,
should be identicaly for the 'dist' subdir.
