Update phpmyadmin to 4.6.4.

pkgsrc changes:

* Overhaul Makefile.
  - Remove use of INSTALL_DIRS and simplify install process.
  - Utilize pkgsrc SUBST_*.
  - Stop other pkglint warninggs.
* Drop some dot files from installation.
